Student Hourly Employment Information

 

NEW EMPLOYEE FORMS

All new employees must complete the following:

  1. Student Employee Form (Instructions for completing the form)
  2. W-4 Form (UW1389-W4) (International Student Employees MUST use GLACIER- the nonresident Alien Tax Compliance System)
  3. Direct Deposit Authorization Form
  4. Electronic I-9 - completed after bringing in legal presence documents to ASM office (See last page of I-9 instructions for list of acceptable documents – no need to complete paper I-9)

STUDENTS MUST TURN IN ALL COMPLETED FORMS TO THE ASM OFFICE LOCATED ON THE 4TH FLOOR OF THE SAC, ROOM 4301

TIMESHEET INFORMATION:

If you forgot to log hours for a pay period, you are able to fill out a prior period timesheet.

This form is ONLY to be used if you are turning in hours from a previous pay period.  The timesheet needs to be signed by the student and supervisor and turned into the ASM Office.  (Instructions for completing prior period time sheet)
